Fit Note Requests

To improve patient safety and to assist clinicians with administration time we will be increasing admin availability on Patchs to enable fit note requests to be received, direct to the Practice. Please register yourself with a Patchs account to allow a request for a fit note to be received and processed within the allocated Practice procedural time.

To access and register for the Patchs platform go to the home page and follow the instructions. The reception team can offer advice and support too.
